This agreement contains information about the Young Men’s Christian Association of Greater Indianapolis Facility Memberships.

Modifications to the Agreement

We may modify this Agreement at any time by posting a revised version on this YMCA of Greater Indianapolis website (http://indy.recliquecore.com/agreements) (“Site”). By continuing to utilize our services after any modifications are made to this Agreement, You acknowledge and agree that it is your responsibility to review these Terms of Use from time to time and to be aware of any such changes.

Terms and Agreements

  • 24 Hour Cancellation – A 24-hour cancellation notice is required or the credit for the session will be charged.
  • Client Health Warranty – Client represents that he/she has consulted with his/her private physician concerning the program, the details of which have been described to his/her physician and the use of the YMCA equipment and/or facilities will not be detrimental to his/her health or well-being.
  • Disclosure – The personal training membership contract is fully integrated and constitutes the entire and exclusive agreement between the parties. Any promises or representation pertaining to the agreement, which is not written in this agreement, are invalid. This agreement may be modified in writing by a YMCA      Member Services representative only. The client and YMCA of Greater Indianapolis employees are not permitted to make any independent agreement with the client regarding Personal Training. If such an agreement is made both the client’s membership and representative’s employment will be subject to termination.
  • Personal Training Usages – I understand all training sessions must be used within the 4-week training month, any sessions unused will be lost.
  • Session Rollover – Limited rolled over is allowed, with prior approval by the wellness director, due to medical or personal emergency or traveling lasting less than 30 days. I understand when rolling over sessions, my EFT draft will remain active. Any unused roll over sessions will expire in 90 days.
  • Hold Privilege – I understand that in the event of a medical or personal emergency or extended travel lasting more than 30 days, I must hold my sessions in advance by notifying my personal trainer. Personal Training holds will be available for a period of 30-90 days per year, and must be requested 48 hours before the 1st of the month.
  • Partner Training – I understand Partner Training must be completed by two individuals. If one individual cancels a session and the session is held, that individual’s credit for the session will be used. In the case where one individual terminates the contract, the entire contract will be terminated, unless the remaining individual fills the “buddy” position. All other terms and conditions apply.
  • PT Service – I understand, if necessary, during this agreement, a different trainer can be assigned to me. If a YMCA employee ends their employment at YMCA, another YMCA employee will complete my sessions and refunds are not warranted.
